Reminder that we have revamped the process to better routinize it for all of us.
TWO steps:
- FILL THIS SIMPLE FORM OUT (summer: most TG’s, if not a TG for summer, will move to GE status so if you want summer coverage I’d mark it)
- WE WILL EMAIL YOU THE INVOICE: you will be invoiced for the total of the terms you marked on the form, but you do NOT have to pay the big lump sum at once!!–you can pay for a single term, or divide the total over twelve months of payments, or pay for the year at once! We just need the total paid by the end of summer.
IF you WANT to either ADD/DROP someone to/from your plan, you can use dependent add/drop form here.
(If you already have a dependent on the plan and are simply continuing the year with them, NO NEED to fill out this form! Only if you are changing your dependents from last year!)
