Do you have a dependent you’d like to add your health care plan?
February 16th through March 15th is Open Enrollment.
Just stop by the GTFF office Monday through Thursday anytime between 9 and 4, fill out a new enrollment form and pay the appropriate premium amount listed below. Payments can be made by check, debit/credit card, or payroll deduction. If you’re adding a Domestic Partner you’ll also need to complete an Affidavit of Domestic Partnership (available at the GTFF office or on this website under the health care tab).
Coverage for new dependents will take effect March 16th, 2012.
Rates are as follows:
Spouse/partner: $273.57 per term
Child(ren): $83.79 per term
Family: $358.62 per term
