HOW to sign up (if you are already on the GTFF insurance):
1) Go to http://www.pacificsource.com/
2) On the top “Find a Doctor or Drug” drop down menu, select “Find a Dentist or PCD”
3) Click on “Find a Primary Care Dentist (PCD) link
4) In the city search box, enter your city (e.g., Eugene, OR)
-In the “Plan or Network” menu, select “Dental Advantage Essentials”
-Click “Find providers”
5) Browse the dentists and pick one that suits your needs
6) When you find a dentist, click on the “Set as my PCD” button (to the right)
7) Enter in your information into the brief form (your ID# is on your insurance card) and hit “Submit”

*This PCD will take effect 09/16/14. Until then, we are under the older dental coverage, and you can see whatever dentist you want. You can always change your PCD later. If you do change PCD’s, your new PCD takes effect on the 1st of the next month.
